Forms

1.  All applicants for a non-immigrant visa, regardless of age or purpose of travel, must complete form:

DS-156: NONIMMIGRANT VISA APPLICATION in English
or
DS-156 DEMANDE DE VISA DE SEJOUR TEMPORAIRE en français

These are electronic forms which must be completed online and printed with the barcode.

2.  In addition, male applicants age 16-45 must also complete form:

DS-157 SUPPLEMENTAL NONIMMIGRANT VISA APPLICATION in English
(Adobe Acrobat format)
or
DS-157 FORMULAIRE COMPLEMENTAIRE DE DEMANDE DE VISA NONIMMIGRANT en français
(Microsoft Word format)

3.  Students and exchange visitors must fill out DS-158 Contact Information and Work History for Nonimmigrant Visa Applicant

4. Applicants for an E-1 Treaty Trader or E-2 Treaty Investor Nonimmigrant visa must complete DS-156E

Other forms or documents may be required depending on the type of non-immigrant visa in question.

VISA APPLICATIONS

Important notice to visa applicants

In preparation for the introduction of the web-based visa application, non-immigrant visa applicants will be required to use the Electronic Visa Application Form as of November 1, 2006.  Handwritten or typed versions of the DS-156 will no longer be accepted.

The Electronic Visa Application Form (EVAF) is available at http://evisaforms.state.gov/. Applicants in Luxembourg can use the English, French or German version, but must provide answers in English. It is important to print the third page including the data in barcode format with the completed DS-156.

The Embassy does not provide internet connection to visa applicants. Following internet cafés provide free internet access and use of a printer in Luxembourg City (most require users to register first and to pay for printed copies):
