When applying for a first time passport at the age of 16 or older:

- use form DS-11
- the passport fee is $100
- you must apply in person
- you must submit proof of US citizenship (birth certificate, Consular report of birth abroad, certificate of citizenship, naturalization certificate)


When applying for a first time passport/passport renewal for a minor under the age of 16:

- use form DS-11
- the passport fee is $85
- minor child must be present
- both parents must sign the application at the Embassy (if this is not possible, a notarized statement of consent may be accepted)
- bring the child's local or U.S. birth certificate or Consular report of birth abroad
- for a first time application, you must submit proof of US citizenship (birth certificate, Consular report of birth abroad, certificate of citizenship, naturalization certificate)


When applying for passport renewal at the age of 16 or older:

- your previous passport must have been issued when you were 16 or older, within the last 15 years, not be damaged and must be submitted with your application.
- use form DS-82
- the passport fee is $75


Statement of Consent DS-3053

This form should be used by a parent or guardian who needs to provide written consent for a minor's passport application and is not able to appear in person with the child.
The form must be notarized. It is possible to have it notarized at the Embassy (no fee) or at the 'Administration communale' of your place of residence in Luxembourg.


Application for additional visa pages DS-4085

This form should be used to add visa pages to a previously issued, currently valid U.S. passport. There is no fee for this service and the passport with the additional visa pages can usually be picked up the same day. It is possible to add additional visa pages twice to one passport book.

Statement regarding lost or stolen passport DS-64

This form should be submitted along with the form DS-11 when you apply in person for a new passport as your previous passport has been lost or stolen.

U.S. Passport Re-Application DS-5504

This form should be used for making a data change for a previously issued, currently valid U.S. passport WITHIN one year from the date of issue:
- When there is a change in your identifying information, such as a name-change due to marriage or by court order (a marriage certificate or court order must be submitted)
-To extend the validity of certain previously limited passports; or
-To correct a Passport Services’ printing error.

There is no fee for this service.
